The Office of 制度的有效性 (OIE) will serve as 云顶集团’s survey clearinghouse. All surveys intended for distribution to any members or prospective members of the college community (such as applicants, students, faculty, staff, board members, and alumni) must be vetted by the Institutional Survey Committee using the procedures 下面描述.
Please note that approval by OIE to administer a survey is not a substitute for a review by the Institutional Review Board (IRB). 调查是学术研究的一部分 research involving human subjects must receive IRB approval. 世界动物卫生组织的调查批准是 separate from the human subject protection requirements of the IRB.
工作人员 or faculty who wish to conduct surveys of 云顶集团 students, alumni, employees, or external partners must have their surveys registered and on the Survey Calendar. This is done by submitting a 检验登记表格 which is reviewed by the OIE and/or Institutional Survey Committee (ISC) as appropriate.
Please note that all data and information collected from surveys conducted by administrative offices, faculty committees, and other college committees are the property of 云顶集团 大学. Upon completion of your survey, you must submit survey 结果, data, and 向世界动物卫生组织提交的任何报告都应存档. Sharing 结果 enhances institutional knowledge, informs future assessment, and helps 云顶集团 meet effectiveness reporting needs. 完成 survey 结果 will be available to the 云顶集团 community as necessary.

Prior to submitting your registration form:

The Institutional Survey Committee will review your application and OIE will respond within 7 business days of the date the proposal was received. Please be aware that if questions on the survey are related to a specific unit, department, role or individual, the supervisor or unit representative will be notified. Further action may be necessary by the responsible party.
